WebTypically AHTRs are hemolytic reactions that occur within 24 h of transfusion and lead to intravascular (rarely extravascular) hemolysis secondary to ABO incompatibility, but may result from other blood group incompatibilities. In contrast, DHTRs typically result in extravascular hemolysis. WebHemolysis is extravascular. The direct antiglobulin (direct Coombs) test establishes the diagnosis and may suggest the cause. Treatment depends on the cause and may include corticosteroids, splenectomy, IV immune globulin, immunosuppressants, avoidance of triggers (eg, cold), and withdrawal of drugs.
